Position Description:

Lake County Board of DD/Deepwood is seeking a full time Habilitation Manager/QIDP responsible for initiating, authorizing, developing and coordinating the individuals Person Centered IP's. . The QIDP is responsible to ensure compliance with Licensure and Medicaid regulations consistent with applicable Board and Residential policies and procedures. Ensures that Habilitation program records are maintained in accordance with Federal Medicaid Standards and DODD rules and regulations.


Duties:

  • Directly monitors and supervises habilitation related staff in the performance of the assigned duties.
  • Ensures that there is monitoring of direct service staff in the implementation and documentation of Individual program IP services outlined in the IP’s, including weekly observation and assessment of the residential dining experience.
  • Applicant will work flexible and variable (40 hours per week).


Requirements & Preferences:

Qualifications:

  • Bachelor's Degree
  • Eligible to meet Federal QIDP qualifications
  • At least two years of experience in DD field
  • Possess a current Ohio Driver License in good standing


Compensation:

Commensurate with experience and qualifications of candidate
